Overview and Recommendations

Background

  • • is a hematologic malignancy caused by clonal proliferation and survival of neoplastic plasma cells within the bone marrow.
  • •Mortality estimates across 15 Latin American countries from 2008–2019 were heterogeneous; Chile (4.12), Uruguay (4.11), and Costa Rica (3.57) per 100,000 persons had the highest reported rates, and US Hispanic mortality was 3.8 per 100,000 persons.
  • •Multiple myeloma is extremely rare in children and young adults; a systematic review identified 42 patients aged ≤25 years with a median age of 17 years and a male-to-female ratio of 1.47:1.
  • •Each 5‑kg/m² increase in body‑mass index was associated with 2% higher incidence and 3% higher mortality from multiple myeloma in prospective cohorts.
  • •Monoclonal gammopathy of undetermined significance (MGUS) is the earliest stage of monoclonal gammopathy and may progress to symptomatic multiple myeloma.
  • •Familial aggregation supports inherited susceptibility; first‑degree relatives have a 2‑ to 4‑fold higher risk, and pathogenic germline DNA‑repair variants occur in 9.1% of sporadic and 18% of familial cases.

Evaluation

  • •Suspect symptomatic multiple myeloma when progressive bone pain, fatigue, anemia, renal dysfunction, hypercalcemia, or osteolytic disease occur together.
  • •Obtain a bone‑marrow aspirate and trephine biopsy; integrate morphology, plasma‑cell immunophenotype, light‑chain restriction, flow cytometry, and cytogenetics.
  • •Demonstrate plasma‑cell lineage with CD138 or CD38 and MUM1; establish clonality with κ and λ light‑chain stains or flow cytometry.
  • •Perform myeloma FISH for del(17p), t(4;14), t(14;16), t(14;20), 1q gain/amplification, del(1p32), and MYC rearrangement; TP53 mutation requires targeted sequencing.
  • •Use whole‑body low‑dose CT for baseline skeletal staging, whole‑body MRI for marrow assessment, and [18F]FDG PET/CT for active and extramedullary disease.
  • •Stage with the International Staging System (ISS), Revised ISS (R‑ISS), Revised Second ISS (R2‑ISS), and Consensus Genomic Staging (CGS) for prognostic risk assessment.
  • •CGS high‑risk criteria include: TP53 pathway abnormality (del(17p) with cancer clonal fraction >20% and/or TP53 mutation), IgH/chromosome‑1 co‑lesion, 1p32 biallelic or compound lesion, or serum β2‑microglobulin ≥5.5 mg/L with creatinine <1.2 mg/dL.
  • •Measurable residual disease (MRD) by next‑generation flow or clonotype‑based NGS at sensitivity ≥10⁻⁵ predicts progression and survival more deeply than conventional serology.
  • •ctDNA is investigational; in 84 patients with 292 longitudinal samples, detectable ctDNA had a positive predictive value of 88.9% and specificity of 98.4% for impending relapse, with a hazard ratio of 11.5 for progression or death.
  • •Targeted MRI of the symptomatic region for new pain with stable biochemical markers had 96% sensitivity and 90% specificity; degenerative disease and fractures were more common than progressive myeloma.

Management

  • •Treat active multiple myeloma with systemic therapy; NCCN states management must be individualized, and ASCO recommends quadruplet induction for transplant‑eligible and suitable transplant‑ineligible patients.
  • •Transplant‑eligible induction: daratumumab 1800 mg SC, bortezomib 1.3 mg/m² SC, lenalidomide 25 mg PO on days 1–14, and dexamethasone 20–40 mg PO weekly in 21‑day cycles for four cycles before stem‑cell collection and autologous transplantation.
  • •Post‑transplant maintenance: lenalidomide 10–15 mg PO on days 1–21 of a 28‑day cycle; consider adding daratumumab, carfilzomib, and/or dexamethasone for selected high‑risk disease.
  • •Transplant‑ineligible fit patients: continuous daratumumab–bortezomib–lenalidomide–dexamethasone; frail patients receive frailty‑adapted daratumumab–lenalidomide–dexamethasone guided by the revised Myeloma Comorbidity Index (R‑MCI).
  • •Relapsed/refractory disease: choose a triplet containing an active proteasome inhibitor, immunomodulatory drug, anti‑CD38 antibody, selinexor, or another non‑cross‑resistant agent; avoid drugs to which the disease is refractory.
  • •For triple‑class‑exposed or refractory disease, use ciltacabtagene autoleucel, idecabtagene vicleucel, teclistamab 1.5 mg/kg SC weekly, elranatamab 76 mg SC weekly, linvoseltamab 200 mg IV weekly, or talquetamab 0.4 mg/kg SC weekly or 0.8 mg/kg SC every 2 weeks.
  • •Give bone‑directed therapy with zoledronic acid 4 mg IV every 4 weeks or denosumab 120 mg SC every 4 weeks for active osteolytic disease; denosumab is favored when renal impairment limits bisphosphonate use.
  • •Radiation: 8 Gy in a single fraction for a painful focal osteolytic lesion; 20 Gy in 5 fractions or 30 Gy in 10 fractions for persistent pain, bulky extramedullary disease, impending fracture, or spinal cord compression.
  • •Geriatric patients: perform a comprehensive geriatric assessment; frailty was present in 22% of adults aged ≥60 years and correlated with increased high‑grade nonhematologic toxicity; use R‑MCI to attenuate treatment intensity.
  • •Renal impairment: do not exclude eligible patients from CAR‑T or bispecific‑antibody therapy solely because of renal impairment; intensify monitoring for cytopenias, acute kidney injury, and early infection.
  • •Use infection prophylaxis, vaccination, and immunoglobulin assessment throughout treatment; BCMA bispecific and CAR‑T therapy can cause prolonged plasma‑cell depletion, hypogammaglobulinemia, and opportunistic infection risk.
  • •Monitor response with monoclonal‑protein studies, marrow MRD by validated next‑generation flow or sequencing at sensitivity ≥10⁻⁵, and functional imaging when indicated; MRD‑directed de‑escalation remains investigational.
