Atrial Fibrillation

Atrial fibrillation is a progressive supraventricular arrhythmia requiring a multipronged approach: stroke prevention with DOACs (apixaban preferred), symptom control via rate or rhythm management, and intensive risk factor modification. Early rhythm control reduces adverse cardiovascular outcomes, and catheter ablation improves survival in HFrEF. Anticoagulation decisions should be driven by CHA₂DS₂-VASc score, not rhythm status, and should be continued indefinitely after ablation in higher-risk patients.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, affecting over 10 million US adults and doubling mortality risk through stroke and heart failure. Diagnosis requires ECG documentation of ≥30 seconds of irregularly irregular rhythm with absent P waves. Management rests on three pillars: stroke prevention with direct oral anticoagulants (DOACs), symptom control via rate or rhythm strategies, and aggressive risk factor modification. Early rhythm control within one year of diagnosis reduces cardiovascular events, and catheter ablation improves survival in heart failure patients.

Overview and Recommendations

Background

  • Atrial fibrillation (AF) is a supraventricular tachyarrhythmia defined electrocardiographically by the replacement of consistent P waves with rapid, irregular fibrillatory waves and an irregularly irregular ventricular response when atrioventricular conduction is intact. It affects approximately 37.6 million people worldwide, with prevalence rising steeply with age: <1% under 55 years to >10% in those ≥80 years.
  • AF doubles the risk of all-cause mortality and increases stroke risk fivefold, approximately 20% of all ischemic strokes are attributable to AF. The arrhythmia also predisposes to heart failure, cognitive decline, and tachycardia-induced cardiomyopathy when ventricular rates are persistently high.
  • Classification by temporal pattern includes first diagnosed, paroxysmal (self-terminating within 7 days), persistent (>7 days), long-standing persistent (>12 months), and permanent (rhythm control no longer pursued). The 2023 ACC/AHA staging system further defines Stage 1 (at risk), Stage 2 (pre-AF with atrial pathology), Stage 3 (documented AF), and Stage 4 (permanent AF).
  • Hypertension is the most prevalent modifiable risk factor, with a population-attributable fraction of ~20%. Obesity (each 5 kg/m² BMI increase raises risk 1.3-1.5-fold), obstructive sleep apnea, diabetes, heart failure, chronic kidney disease, alcohol (heavy consumption), and physical inactivity are established drivers. Age is the strongest non-modifiable risk factor, hazard doubles each decade after age 50.

Evaluation

  • Suspect AF in any patient presenting with palpitations (sudden onset of rapid, irregular, or fluttering heartbeat), dyspnea, exertional intolerance, fatigue, dizziness, presyncope, or syncope. Up to 40% of patients are asymptomatic, discovered incidentally on routine examination or after a thromboembolic event.
  • The classic physical finding is an irregularly irregular pulse with variable pulse amplitude. S1 heart sound varies in intensity, and the jugular venous pulse loses normally occurring a-waves. Always examine for signs of heart failure (elevated JVP, pulmonary crackles, peripheral edema), hyperthyroidism, obesity, and sleep apnea (crowded oropharynx, large neck circumference).
  • Gold-standard diagnosis is a 12-lead ECG showing absolute R-R interval irregularity, absence of discrete P waves with fibrillatory waves, and narrow QRS (unless pre-existing conduction disease). Arrhythmia must persist ≥30 seconds for the diagnosis of clinical AF.
  • If paroxysmal AF is suspected but the initial ECG is non-diagnostic, proceed to ambulatory monitoring: a 24-hour Holter for frequent symptoms, 7-day patch monitor for higher yield, or 30-day event recorder/insertable cardiac monitor (ICM) for infrequent episodes. After cryptogenic stroke, ICM detects AF at 6 times the rate of conventional follow-up within 6 months.
  • Order laboratory studies in all new AF: thyroid function (TSH), electrolytes (K+, Mg), renal function, complete blood count, and natriuretic peptides (BNP/NT-proBNP). Consider toxicology screen if alcohol or stimulant use is suspected.
  • Transthoracic echocardiography is indicated for all patients to assess left atrial size (volume index ≥34 mL/m² correlates with AF persistence), left ventricular systolic and diastolic function, valvular disease, and pericardial pathology.
  • Transesophageal echocardiography (TEE) is reserved before cardioversion or catheter ablation if anticoagulation has not been therapeutic for ≥3 weeks. Intracardiac echocardiography (ICE) is a growing alternative with noninferior safety and less major bleeding from transseptal puncture.
  • Diagnostic criteria require ECG documentation of ≥30 seconds of AF. The workup should confirm the arrhythmia, characterize its pattern (first-detected, paroxysmal, persistent, long-standing persistent), identify precipitants (thyroid, alcohol, surgery, infection, MI, pericarditis), assess structural substrate, and evaluate for concomitant conditions (hypertension, sleep apnea, obesity, diabetes, CAD, heart failure).
  • Use the EHRA score to grade symptom burden: Class I (asymptomatic) through Class IV (disabling). Remember that symptom absence does not predict low thromboembolic risk, silent AF carries stroke risk comparable to symptomatic paroxysms.

Management

  • First, assess hemodynamic stability. If unstable (hypotension, acute myocardial ischemia, pulmonary edema, altered mental status due to rapid ventricular response), proceed urgently to synchronized electrical cardioversion starting at 100-200 J biphasic.
  • For hemodynamically stable patients, decide between rate control and rhythm control based on symptom burden, AF duration, and comorbidities. Early rhythm control within one year of diagnosis reduces cardiovascular events (HR 0.79, NNT = 37 at 5 years).
  • Acute rate control: beta-blockers ( 5 mg IV every 5 min ×3 doses, then 25-50 mg PO BID; target 200 mg daily) or nondihydropyridine calcium channel blockers ( 0.25 mg/kg IV, then 5-15 mg/h infusion; max 360 mg PO daily) are first-line. Avoid diltiazem if LVEF <40%.
  • For rate control in HFpEF or permanent AF with HF symptoms, consider 0.0625-0.125 mg PO daily (target serum level 0.5-0.9 ng/mL). RATE-AF trial showed superior symptom improvement versus in this population.
  • Heart rate target for rate control: <80 bpm at rest (class IIa) or <110 bpm with moderate exertion (class IIb). Beta-blockers do not reduce mortality in patients with concomitant HF and AF (HR 0.97).
  • For pharmacologic cardioversion without structural heart disease: 2 mg/kg IV or 300 mg PO, or 2 mg/kg IV or 600 mg PO. For structural heart disease: 1.2-1.8 g PO in divided doses over 24 hours.
  • Electrical cardioversion with biphasic synchronized shock (100-150 J initial) requires therapeutic anticoagulation for ≥3 weeks or a TEE excluding left atrial appendage thrombus.
  • Initiate oral anticoagulation based on CHA₂DS₂-VASc score: recommend DOAC for men with score ≥2 and women with score ≥3. Direct oral anticoagulants are preferred over warfarin: 5 mg BID (reduce to 2.5 mg BID if age ≥80 years, weight ≤60 kg, or creatinine ≥1.5 mg/dL); 20 mg daily with food (15 mg if CrCl 15-49); 60 mg daily (30 mg if weight ≤60 kg, CrCl 15-49, or P-gp inhibitor).
  • After acute ischemic stroke with AF, initiate a DOAC early (≤4 days) to reduce recurrent ischemic stroke without increasing intracranial hemorrhage (CATALYST meta-analysis OR 0.70, NNT = 111).
  • For patients with AF and stable CAD, use OAC monotherapy (DOAC alone), the EPIC-CAD trial showed 56% relative risk reduction in composite outcomes versus dual antithrombotic therapy (NNT = 12).
  • Do NOT use for stroke prevention in AF, it is inferior to OAC and carries similar bleeding risk. Avoid non-dihydropyridine CCBs (verapamil, diltiazem) in HFrEF. Avoid as first-line for pharmacologic cardioversion without structural heart disease.
  • Risk factor modification is essential: target ≥10% weight loss in obese patients (reduces AF recurrence after ablation by nearly half), blood pressure <130/80 mm Hg, screen for and treat obstructive sleep apnea, and recommend alcohol abstinence (reduces recurrence HR 0.55).
  • Refer for catheter ablation (pulmonary vein isolation) as first-line therapy for symptomatic paroxysmal AF and for selected persistent AF patients. In HFrEF, ablation improves survival and reduces HF hospitalizations (CASTLE-AF: NNT = 7 for primary outcome).
  • Continue anticoagulation indefinitely after successful ablation if CHA₂DS₂-VASc ≥2 (men) or ≥3 (women), regardless of rhythm success. Discontinuation may be considered only after shared decision-making in lower-risk patients.
  • Left atrial appendage occlusion is a reasonable alternative for stroke prevention in patients with clear contraindications to long-term anticoagulation.
  • AV junction ablation with biventricular pacing (CRT) is a rescue strategy for patients with refractory rapid ventricular rates despite maximal medical therapy, APAF-CRT showed reduced mortality versus pharmacologic rate control (NNT = 6 at 2 years).

  • EAST-AFNET 4, Early rhythm control within 1 year of AF diagnosis reduces CV death, stroke, HF hospitalization, or ACS (HR 0.79); NNT = 37 at 5 years.
  • CASTLE-AF, Catheter ablation in HFrEF with AF reduces all-cause mortality (13.4% vs 25.0%) and HF hospitalization; NNT = 7 for primary outcome.
  • RATE-AF, In permanent AF with HFpEF, digoxin improved symptoms and NYHA class versus bisoprolol with fewer adverse events; NNT = 2 to improve one NYHA class.
